Authorities in Ohio are investigating the death of a 7-month-old baby boy who died last week after hospital staff accidentally flushed his system with alcohol instead of saline.

On August 21, Tressel Meinardi was scheduled to have open-heart surgery at Cincinnati Children’s Hospital around 7:30 a.m.

According to a Facebook page that was set up to memorialized Tressel, while the infant was in the operating room, hospital staff spoke with his family several times, but it was not until 10:30 p.m. that they informed the family about the mistake.

The alcohol caused Tressel’s organs to fail, which caused his body to double in size.

After Tressel’s death, the hospital sent a memo to its employees outlining the hospital’s policy when a mistake is made.

“In cases where there has been an error,” the memo says.  “We accept responsibility, admit the error, apologize for it and explain what happened.”

Tressel was buried last week in his hometown of Richmond, Indiana.

A Connecticut woman, who lied about her credentials in order to get a job teaching autistic children in Fairfield County, pleaded guilty today in court to charges of larceny in the first degree, forgery in the second degree and a count of criminal impersonation.

Stacy Lore admitted to charging Norwalk and Weston school districts and families almost $500,000 for her services as an autism therapist, a job she was not qualified for.

Lore, the owner of Spectrum Kids LLC, charged Norwalk schools $155,781 in 2007 and 2008 for her work with autistic pre-school children.  She also charged families in the area more than $30,000 for private therapy.

Lore, who was arrested at her home in Carmel, New York, claimed she held a doctorate from New York University, however, authorities say she only holds a GED from a vocational school in NY.

A criminal investigation began in 2008 after parents became concerned about Lore and the therapeutic programs she had designed for their children who were diagnosed with autism.  Dissatisfied with the response they received from school officials, they contacted the Behavior Analyst Certification Board in Florida.

In August 2008, the parents received written confirmation from Christine Ratcliff, COO of BACB, Inc., stating that Lore was not a certified therapist.  The parents then passed the information over to Norwalk school officials.

Parents said although they provided the school with the information, the district kept Lore on their payroll.  They said it wasn’t until March 2009 that the district contacted the police.

Authorities charged Lore after they were unable to verify her credentials from NYU.

A 4-year-old NYC girl was found dead early this morning with her arms and legs tied to a crib in her Brooklyn home.

Police were called to the Bedford-Stuyvesant home of 30-year-old Carlola Pierce around 7:30 a.m., where they found Marchella Pierce weighing only 15 pounds and tied with rope to the cribe in the filthy apartment.

Police said Marchella’s “legs were like sticks,” and you were able to see her ribs.  They said the girl appeared “underdeveloped with undeveloped lungs.”

Pierce told police when she checked on her daughter around 4:00 a.m. she was perfectly OK, but when she went back around 6:30 she found her unconscious.  She didn’t however explain to police why she waited an-hour-an-a-half to call 911.

Authorities say the little girl had bruises on her ankles, arms and wrists and there was an oxygen tank close to her but it was not connected.

It is believed that Children’s Services had visited Pierce’s home on two different occasions. Details of those visits were not available.

Pierce has two other children, one 5 and the other 9-months-old.

Charges have not been filed against Pierce, but police are still investigating the incident.

It is unclear whether her two other kids were taken into custody.

According to RadarOnline.com, Teresa Giudice’s sister-in-law accepted Andy Cohen’s offer to be in the third season of “The Real Housewives of New Jersey.”

The online gossip website says that Melissa Gorga, who lives larger than Teresa, minus foreclosure, will replace Dina Manzo, who left the popular reality show after Danielle Staub tried to have her daughter Lexi taken away.

Gorga is married to Teresa’s brother, Giuseppe, who is a real estate developer.

She lives in a $4 million home and has two live-in nannies in Montville.

The cast of NJ are threatening to go on strike if they do not get a raise in salary, profits from DVD and merchandise sales.

A Canadian high school teacher has been arrested and charged with sexually exploiting at least one of his students in his Toronto home over the summer.

Authorities say the incident occurred in June.

It is believed that Agincourt Collegiate Institute teacher Peter Floro assaulted the student on three different occasions.

Authorities believe he also assaulted other students.

School officials have not made a statement as to whether he will continue to teach at the school, where he has been a teacher for the last four years.
