Authorities in Ohio are investigating the death of a 7-month-old baby boy who died last week after hospital staff accidentally flushed his system with alcohol instead of saline.
On August 21, Tressel Meinardi was scheduled to have open-heart surgery at Cincinnati Children’s Hospital around 7:30 a.m.
According to a Facebook page that was set up to memorialized Tressel, while the infant was in the operating room, hospital staff spoke with his family several times, but it was not until 10:30 p.m. that they informed the family about the mistake.
The alcohol caused Tressel’s organs to fail, which caused his body to double in size.
After Tressel’s death, the hospital sent a memo to its employees outlining the hospital’s policy when a mistake is made.
“In cases where there has been an error,” the memo says. “We accept responsibility, admit the error, apologize for it and explain what happened.”
Tressel was buried last week in his hometown of Richmond, Indiana.
